Participant desires to participate in Compound Fitness’s 2024 Kickball Season played at American Legion Field in Brook Park, OH. In consideration of being allowed to participate with the program and related events/activities, the undersigned acknowledges, appreciates, and agrees that:

1. My participation in this activity is purely voluntary.
2. I am fully aware of the fact that there are special dangers and risks associated with participation in this activation, including but not limited to the potential for falls, slips, sprains, broken bones, extreme physical contact with other participants or outbursts of rage by other players, coaches, or umpires. Serious injury may also occur as a result of certain playing conditions such as potholes, standing water on fields, along with humidity, heat, cold, and other weather conditions inherent with games played outdoors. Serious injury may also occur as a result of certain playing conditions inherent with games played indoors. Serious injury or sudden death may also occur as a result of improper use of equipment.
3. Participation in this activity includes possible exposure to an illness from infectious diseases including but not limited to MRSA, influenza, and COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist.
4. Kickball is a fun team and community outreach. I vow to have the best time, not yell at the umpire because I know we are all human, or get too upset if my team loses. I will have good sportsmanship and shake the other team's hand after the game has concluded.
5. I understand that if I fail to fully comply with the terms and protocols established by the provider or the commitments I have agreed to in the release, I am subject to immediate removal from participation.
